What Is Adam’s Apple Reduction?

Men typically have a more prominent Adam’s apple than women. Adam’s apple reduction, or chondrolaryngoplasty is the surgical procedure for male transitioning to female to make the Adam’s apple look more feminine and less prominent. It is done by reducing thyroid cartilage through an incision in the throat.

How Is Adam’s Apple Reduction Performed?

The procedure for Adam’s apple reduction involves making an incision across the bottom of the Adam’s apple. Once this is done, the throat muscles are held apart while the thyroid cartilage is shaved down so that the throat looks smoother without having a protruding Adam’s apple. Once this is accomplished, the incision is closed with sutures to allow the incision to heal with a scar remaining for several weeks.

As one of the most commonly performed gender affirming procedures, Adam’s apple reduction is performed on an outpatient basis and usually takes about one to two hours to complete. Although it is fairly simple to perform, great care must be taken not to “over-shave” the cartilage.

Preparing for Adam’s Apple Reduction

Adam’s apple reduction is done as part of gender affirmation and requires a detailed psychological profile to make sure patients are capable of handling the mental and emotional effects of having this procedure. Additionally, as this is considered major surgery several recommendations for patients are as follows:

  • Undergo pre-operative testing
  • See your physician for medical clearance
  • Adjust or discontinue current medications
  • Take certain medications
  • Stop smoking
  • Avoid aspirin, anti-inflammatory drugs, and certain herbal supplements that may thin your blood.
  • Make travel arrangements for someone to drive you to and from surgery
  • Arrange for someone to stay with you the first night following surgery

Recovery from Adam’s Apple Reduction

Swelling typically peaks within the first 24 to 48 hours and takes about six weeks to resolve itself. Dr. Taylor suggests that his patients gently rub the incision to reduce the chances of scar tissue forming and causing a noticeable mark. For about seven to ten days, there will be some discomfort especially with talking or swallowing.

The scar may be visible for several months, but as time progresses, the scar diminishes to the point that it is almost invisible within a year.
